Cosmetic Surgery · Major US metro
A cosmetic surgery practice that had lost over 90% of its Google rankings in a year. We stopped the slide, then reversed it.
A busy cosmetic surgery practice in one of the most expensive markets in the country had watched its Google rankings fall steadily for twelve months, losing more than 90% of them, while paying an agency the whole time. We made the case to replace that agency, took over on a retainer of just over $6,000 a month, stabilized the rankings, and turned them around. The new patient revenue attributable to organic search has far exceeded the retainer.
The situation
Over the preceding twelve months the practice had lost more than 90% of its Google rankings, steadily, month after month, while its existing agency reported that things were fine. Paid ads were covering the gap at growing cost. The WordPress site had been compromised more than once, procedure pages were thin, and nobody could explain the decline. We had to first convince the practice that the agency, not the market, was the problem, then stop the bleeding before any growth work could start.
What we did
- Diagnosed the causes of the twelve-month decline and made the case, with the data, for replacing the previous agency.
- Stabilized and secured the site, cleaned up the technical foundation, and mapped every procedure to real search demand in the metro.
- Prioritized the procedures with the highest case value first, so early wins showed up in revenue, not just rankings.
- Built a content program around the highest-margin procedures: full procedure pages, cost and recovery guides, and gallery optimization.
- Managed the Google Business Profile and review process alongside on-site work.
- Reported monthly on rankings and consult-driving pages rather than raw traffic.
The outcome
The slide stopped within the first months. Rankings for the practice's core procedures then recovered and kept climbing, consult requests from search grew with them, and the new patient revenue from those consults has covered the retainer many times over. That is the standard we hold every engagement to: the work has to pay for itself several times before it counts as working. Specific rankings, lead counts and revenue figures are shared on a strategy call, out of respect for the client's confidentiality.
Client details and figures may be available on a strategy call. We do not publish results without a practice's written permission.
